Where clinical expertise meets compassionate care.

A Note from Kimberly

Dear Friends and Neighbors,

As the weather cools and the holidays draw near, I find myself thinking about how much we all lean on the people we love, and how important it is that nobody feels alone when health questions come up. After more than 32 years as a nurse, I have learned that the right information at the right moment can change everything. This month, I want to share a few simple ideas to help you feel more prepared and more confident.

With warmth,
Kimberly Wright, MSN, RN

Health Tip of the Month: Get Ready Before You Leave the Hospital

Whether you are heading home from a hospital stay or getting ready for a doctor visit, a little preparation goes a long way. Here is what I tell my own families:

  • Ask early about going home. Starting the day after you arrive, ask, “What needs to happen before I can go home safely?”
  • Write down your questions. Keep a notepad by the bed. No question is too small.
  • Review every medicine. Ask which medicines are new, which have changed, and which you should stop. Ask for the list in writing.
  • Use the “teach-back” method. After the nurse or doctor explains your instructions, say them back in your own words to make sure you understood.
  • Bring someone with you. A family member or friend is a second set of ears. If they cannot be there, ask if they can join by phone.
  • Book your follow-up visit before you leave. Ask for the date, time, and the name of the doctor.
  • Know who to call. Get a phone number for questions after you get home, and ask what warning signs mean you should call right away.

Featured Service: Post-Hospital Care Setup

Coming home from the hospital can feel overwhelming. There are new medicines, new instructions, and appointments to keep track of, all while you are still recovering. That is exactly why I created Post-Hospital Care Setup.

What it includes:

  • An in-home visit. I come to you in Orleans or Jefferson Parish to see your home, your routine, and what you will need to feel safe and comfortable.
  • Medication review. We go over every medicine together, in plain language, so you know what each one is for and when to take it.
  • Understanding your discharge instructions. I help you and your family make sense of what the hospital told you.
  • Follow-up appointments and care coordination. We organize your appointments and I help connect the dots between your doctors, specialists, and caregivers.
  • Support for family caregivers. You will know what to watch for and who to call.

Your loved one deserves a nurse in their corner. I follow you from the hospital door into daily life, so nothing falls through the cracks.

Did You Know?

Many people with high blood pressure feel perfectly fine, which is why it is often called a “silent” condition. The only way to know your numbers is to check them. If you have hypertension, diabetes, COPD, or heart disease, keeping a simple daily log of your blood pressure, blood sugar, weight, or breathing symptoms gives your doctor a clear picture at your next visit and can help catch small changes before they become big problems. Bring your log to every appointment.

Community Corner

Thank you to everyone who has welcomed me into your churches, community events, and conversations. You remind me every day why this work matters. Caring for one another is at the heart of a strong community.

“Bear one another’s burdens, and so fulfill the law of Christ.” (Galatians 6:2)

If you belong to a church or community group that would like to learn more about supporting seniors, I would love to talk with you about our Community Covenant Care Initiative. And if you simply want to say hello, ask a question, or share a story, reach out any time. You are not alone in this.
